1. Be a CollaMouver by creating your account on our website.
2. Ask your Physician or Pharmacist to fill out the below (FACIN) form
3. Your Physician or Pharmacist will provide a list of candidate patients to take part of CMC Support Program
4. Make sure to give your CollaMouver Account email to your Health Care Professional while he’s in communication with us.
5. Once your Health Care Professional give us your CollaMouver account, we will be in contact with you either by Email or Telephone to inform you how to get your CollaMouv® or CollaMouv Plus® Drinkable Shots for 8 Weeks.
